Please be sure to include your full name, mailing address, pets name and any helpful information about your pets condition.  We will ask for details.  It is important for us to know if a wheelchair is the right thing for your pet.

Helpful Hint: 

#4 measurement may be taken with your pet laying on it's side.  Just place the end of the tape or ruler on your pet's front leg (up near the chest wall) and run it back until you touch the FRONT of the rear leg.
